Since 2011, Asha has been delivering a service under a ‘Reablement’ contract, awarded by the Joint Commissioning Unit (Health and Social Care) for Worcestershire.
The Worcestershire Reablement Service grew out of the redesign of day services and provides support to Service Users with mental health problems on the road to recovery.
Referrals to Asha Reablement are received through the Worcestershire Mental Health Hub who receive their referrals from main stream, tier 2, mental health professionals i.e. a consultant Psychiatrists or the Community Mental Health Teams.
The Hub Service helps Service Users to identify the help they need, and then works with them to make referrals the hub to the most appropriate Reablement Bridge Builder organisations
Asha uses a ‘One Stop Shop’ model Asha to provide our bridge building service. This offers a wide range of options for women to equip themselves with the confidence, information, networks, skills and support they need to make a successful transition from intensive 2nd tier clinical services typically provided by community mental health teams, towards more independence and use of ‘main stream’ services.
The Asha Reablement Project has two dedicated part time Support Co-ordinators, both with professional mental health backgrounds; one of whom qualified as Community Psychiatric Nurse and one of whom qualified as a Social Worker and Approved Social Worker
The Worcestershire Reablement Service has been recognised in the Compact Awards for 2011 and commended by the judges as an example of voluntary and public sector organisations working collaboratively to gain better outcomes for service users.
